Terms of Service, Consent & Disclosure

By scheduling and receiving services, I acknowledge, understand, and agree to the following:

Prenatal Care Acknowledgment

• I confirm that I am currently receiving ongoing prenatal care from a licensed physician or healthcare provider.

• I understand that my healthcare provider is solely responsible for confirming my due date, monitoring fetal development, screening for abnormalities, and addressing any medical concerns related to my pregnancy.

• I understand that Premier View Imaging is not a medical provider and does not provide diagnostic, evaluative, or medical ultrasound services.

Nature of Services

• I understand that the ultrasound services provided are elective, non-diagnostic, and for keepsake and bonding purposes only.

• These services do not replace any diagnostic ultrasound, medical exam, test, or treatment ordered or recommended by my healthcare provider.

• I understand that the ultrasound session is limited in scope and duration and focuses only on capturing keepsake images and video.

• Because these services are elective and non-medical, they are not covered by insurance, and full payment is required in advance.

Technician & Image Limitations

• I understand that the ultrasound technician is not a doctor, nurse, or healthcare provider and cannot diagnose conditions, interpret medical findings, or provide medical advice.

• No images or recordings produced during my session will be reviewed or interpreted by a physician or healthcare provider.

• I understand that no guarantees are made regarding fetal position, movement, visibility, image quality, or the ability to visualize specific features.

• Image and video quality may be affected by factors including gestational age, fetal position, body composition, and technical limitations.

Gender Determination

• I understand that any gender determination is not guaranteed, and errors are possible.

• I acknowledge that factors beyond the control of Premier View Imaging may affect accuracy, and no warranty or guarantee is made regarding gender determination.

Health & Safety Disclosure

• I understand that while ultrasound is widely believed to be safe, future research may reveal effects that are currently unknown.

• I accept all risks associated with voluntarily receiving this elective ultrasound.

Client Responsibility

• I understand that I am responsible for contacting my own healthcare provider with any questions or concerns regarding my pregnancy or this ultrasound.

• I certify that I am not receiving this ultrasound in place of standard prenatal medical care.

Media, Images & Content Use

• I understand that all images and video captured during my session may be used by Premier View Imaging for marketing, educational, and promotional purposes, unless I provide written notice otherwise.

• I grant permission for Premier View Imaging to use social media content (photos, videos, reviews, or posts) that reference or tag the business, whether posted by me or others, for promotional purposes without compensation.

• I understand that recording or distributing unauthorized images or video during the session is prohibited.

Communication Consent

• I consent to receive appointment-related communications, images, and videos via email and mobile device.

• I agree to receive email and SMS communications, including surveys, updates, and promotional messages. I understand I may opt out at any time.

Release of Liability

In consideration of the services provided, I hereby release and hold harmless Premier View Imaging, its owners, employees, contractors, and affiliates from any and all claims, liabilities, damages, or causes of action arising from or related to this elective ultrasound, including but not limited to:

• Image quality or limitations

• Gender determination accuracy

• Any perceived or unknown effects of ultrasound exposure

Acknowledgment

By signing or electronically accepting this agreement, I confirm that I have read, understand, and voluntarily agree to all terms stated above.
