RELEASE AND WAIVER OF LIABILITY

Premier View Imaging, LLC provides elective ultrasound services solely for entertainment and viewing purposes. All ultrasound sessions are non-diagnostic in nature and are not intended to replace standard prenatal medical care, diagnostic imaging, treatment, or evaluation provided by a licensed healthcare professional.

By signing below, I acknowledge, understand, and agree to the following:

• I confirm that I am currently under the care of a physician or other licensed healthcare provider for my pregnancy.

• I acknowledge that I have previously undergone, or will undergo, a medical diagnostic ultrasound as prescribed by my healthcare provider and that the elective ultrasound services provided by Premier View Imaging are not a substitute for such care.

• I understand that all ultrasounds performed by Premier View Imaging are provided for entertainment and viewing purposes only and do not replace diagnostic ultrasounds or any medical testing, treatment, or recommendations made by my healthcare provider.

• I understand that although the sonographer performing my elective ultrasound is trained and qualified to operate ultrasound equipment, they are not a physician, nurse, or healthcare provider and are not permitted to interpret images, diagnose medical conditions, or provide medical advice regarding fetal health or my pregnancy.

• I understand that the quality and clarity of ultrasound images depend on multiple factors, including fetal position, gestational age, body composition, and technical limitations, and that Premier View Imaging does not guarantee image quality or the accuracy of any gender determination.

• I acknowledge that Premier View Imaging retains ownership of all photos, videos, and images produced during my ultrasound session and authorize Premier View Imaging to use such content for marketing, advertising, educational, or promotional purposes, without compensation.

• I understand that while ultrasound is widely believed to be safe, future research may reveal risks or effects that are currently unknown.

Release of Liability

In consideration of the services rendered, I hereby release, waive, and hold harmless Premier View Imaging, LLC, its owners, agents, employees, contractors, and affiliates from any and all claims, demands, damages, injuries, losses, or liabilities arising out of or related to this elective ultrasound, including but not limited to any inaccurate determination of fetal gender or perceived effects of ultrasound exposure.
